Patient Details
This patient, 5'4" with a BMI over 30, wanted to enhance her natural curves while maintaining balanced body proportions. Although she already had a well-shaped buttock with good lower pole fullness, she desired better upper buttock projection, improved contour, and a more defined hourglass silhouette.
Patients with a BMI over 30 are often told they are not candidates for a Brazilian Butt Lift, but that is not always the case. While candidacy depends on overall health, body fat distribution, skin quality, and individual anatomy—not BMI alone—many patients with a higher BMI can achieve beautiful, natural-looking results.
Dr. Mihail Climov performed 360° power-assisted liposuction to sculpt the abdomen, waist, flanks, back, bra line, and neck, creating a slimmer, more defined torso. Rather than discarding the harvested fat, it was carefully purified and used for ultrasound-guided gluteal fat grafting (Brazilian Butt Lift).
Instead of simply making the buttocks larger, fat was strategically placed to enhance upper pole fullness, improve projection, and refine the overall shape while preserving the patient's naturally attractive curves. The result is a more balanced waist-to-hip ratio and a youthful, lifted appearance.
This case demonstrates that a successful Brazilian Butt Lift is about proportion, shape, and artistic fat placement—not simply increasing volume. Even for patients with a BMI over 30, thoughtful body contouring combined with precise gluteal fat grafting can create a dramatic improvement in silhouette while maintaining natural-looking results.
Individual results may vary. The decision to perform a BBL is based on a comprehensive evaluation of each patient's anatomy, overall health, and surgical goals rather than BMI alone.
